Five times. Not five percent. Five times.

122,007 adults walked a treadmill at the Cleveland Clinic and were followed for a median of eight years. The least fit quartile died at roughly five times the rate of the fittest group. In the same cohort, low fitness carried more risk than smoking and more than diabetes.

Two things are worth knowing about that number. The comparison group is the top 2.3%, so this is the full width of the range rather than two ordinary people side by side. And cardiorespiratory fitness is the most trainable thing on that list. You cannot train your age or your family history. You can train this one.

Almost nobody has it on paper. Bloodwork, blood pressure, see you next year. None of that reads what the body can actually do.

VO2max is measured with metabolic testing on day one, and on a schedule after that.

44%. That is how much lower the risk of death was in the men who went from unfit to fit.

They took a treadmill test. Five years later they took it again. The men whose score improved were far less likely to have died than the men whose score stayed low.

Every extra minute they lasted on the treadmill was worth about 8%.

Your fitness is not fixed. It moves when you train it. You just have to know the number you are starting from.

The PNOE measures the air you breathe in and out, either during exercise or at rest. Oxygen coming in, carbon dioxide going out, breath by breath.

From that we read total oxygen consumption, the calories you are actually burning while you work, and your heart rate training zones. Not the zones your watch assigned you from your age. The ones your own gas exchange says are yours.

So it tells us two things at once. Where your fitness sits today, and how your program should be structured to move it.

Most people have never had any of it measured. A watch estimates it. A calculator guesses at it. This tests it.

16%. That is how much higher the risk of death is for every 5 kg of grip strength lost.

Grip strength is not a JUST a reading about your hands. It is the quickest whole body strength read there is, and it shows up in the things that keep you independent: opening a jar, doing up a button, holding the rail on the stairs, carrying both bags in one trip. Lower grip also tracks with faster cognitive decline and higher dementia risk.

Grip goes up when you load it. The test takes four seconds on a dynamometer. We take it on day one and again at every Longevity Assessment, so you can see which way your number is going.

"Conner, what are these straps for?"

They're compression cuffs that fill with cold water while you're working. They restrict the blood leaving the muscle, so metabolites accumulate and the muscle fatigues at a load far lighter than it would normally take. Once the endurance fibers fatigue, the nervous system has to recruit type 2 fibers, which usually only engage under heavy weight.

That is the principle behind blood flow restriction training, and it is why people build real strength at 20 to 30% of the load they would otherwise need. It also means the work is available to someone whose knees, spine or cardiac history rules out heavy lifting.

The cooling reduces how much blood the body diverts to the skin to shed heat, leaving more of it available to the working muscle.

Seated, no impact, 21 minutes. Vasper reports that as the equivalent of about two and a half hours of anaerobic exercise. That is the manufacturer's figure, not an independent trial.

This is the bridge between training and recovery. You have done real work, and you step off invigorated rather than wrecked. That's how it leaves me, anyway.

"When you hear that you have osteopenia, which is turning into osteoporosis, you do lose confidence. Maybe you won't go for that hike, or you won't go up that hill. It's always hovering over you."

That's the part of a bone density result nobody prepares you for. Not the fracture. The years of small refusals before one ever happens. The hill you skip. The grandchild you don't lift.

Fear like that doesn't respond to reassurance. It responds to being strong enough that the fear stops making sense.

She is 85% stronger across the Core 4 lifts, leg press, row, horizontal press and pull down, than she was on this date last year. Her bone density has moved in the right direction over the same twelve months.

Strength and balance are what keep you off the floor. Bone is what decides what happens if you end up there. She has been training both.

Two sessions a week as part of our Bones & Balance program. Measured, so you can watch it move.

Members look at me sideways when I tell them the daily target is 100, 150, sometimes 200 grams.

So here is the breakdown.

Total intake across 24 hours is the variable that matters most. But a number that size is a logistics problem, not a willpower problem. What works is finding five to ten meals, shakes and powders you will actually repeat, then spreading them through the day instead of stacking them at dinner.

Per meal, the muscle building response climbs with the dose and then levels off. Thirty grams of quality protein clears that point for most people, and the requirement rises with age rather than falling. Eat less in a sitting and you still get a response, just not the whole one.

Here is the part almost nobody is told. Protein has no storage depot. You bank carbohydrate as glycogen and fat in adipose tissue, and there is no equivalent reserve for amino acids. When intake falls short the body takes what it needs from the tissue you spent decades building. Your skeletal muscle is the reserve.

That is the whole case for eating it consistently rather than heroically.

Your target is set off lean mass, not body weight, which is why we scan before we set the number.

Ask anyone in longevity for the three physical markers that matter most for your physical fitness and you should get the same three: grip strength, lower body strength, and VO2max.

Not just because they're interesting on their own. Because between them they cover most of what your body can physically do and how well it can perform.

Grip is the cheapest read on whole-body strength there is. Lower body strength is whether you get out of a chair without your hands, and whether you get up off the floor at 80. VO2max is how much work you can do before you have to stop, which is the back nine on the course, the last few sets on the tennis/pickleball court, the long set of stairs at your home, the stadium, or on vacation, or the steep hill on the hike you agreed to go on.

Physical fitness extends into every area of your life.

We measure all three markers on a quarterly basis and adjust your program and our recommendations accordingly. Not just measured once at the start of the program to be forgotten about. We measure it quarterly so we can assess the trend instead of a just meaasuring a snapshot.

You can't fill a deficit you haven't found.

What gets measured gets improved. Know your numbers.
