We’re proud to see CAPABLE represented at the Fall 2026 NAACOS Conference!
Amanda Goodenow, Director of the CAPABLE National Center, and Tricia Ford, Care Synergy CEO Elect/Chief Operating Officer – Hospice/CAPABLE are attending the conference, connecting with health care leaders and sharing information about CAPABLE at an informational booth.
CAPABLE (Community Aging in Place – Advancing Better Living for Elders) brings together an occupational therapist, a registered nurse and a handy worker to help older adults address the challenges that can make living safely at home difficult. By focusing on each person’s goals and the barriers in their home environment, CAPABLE helps support greater safety, independence and quality of life.
We’re excited to share this innovative, person-centered approach with health care organizations working to improve outcomes for older adults!

Preventable harm remains one of health care’s most urgent challenges, and community-informed innovation is part of the solution; it takes bold ideas, trusted partnerships, and a commitment to better outcomes. That’s why we’re proud to share that Care Synergy and The CAPABLE National Center have been selected as winners of Elevance Health Foundation’s inaugural Patient Safety Prize, launched through the organization’s Community Action Leadership program.
This support will help us advance our work in fall prevention and accelerate practical, community-informed solutions that improve patient safety and quality of care, especially for underserved communities.
We’re grateful for the opportunity to keep building this work alongside the patients, families, partners, and communities who shape it every day.

When organizations work together, our communities benefit.
Recently, Colorado Visiting Nurse Association and Colorado PACE teamed up to bring a Medical Foot Care clinic to the Colorado PACE Day Center, making it easier for participants to receive important preventive foot care in a familiar, convenient setting.
It’s a simple example of something we see throughout the Care Synergy network: when mission-driven organizations share resources, expertise and a commitment to the people they serve, new opportunities for collaboration can open doors to better care.
Foot health is about more than feet. Regular medical foot care can help identify changes in a person’s health, support mobility and help older adults remain active and independent.
We’re proud to work together with Colorado PACE to help keep our community healthier, one step at a time.

Healthy feet can play an important role in helping older adults stay active, independent and safe.
As we age, foot problems can become more common, and conditions such as diabetes, neuropathy, circulation problems, arthritis and heart disease can make routine foot care especially important. Regular foot assessments can also help identify concerns that may need additional medical attention.
Colorado Visiting Nurse Association (CVNA) offers Medical Foot Care Clinics throughout the Denver metro area, with several clinics each month, including locations in Boulder, Castle Rock and Brighton.
During a 30-minute appointment, a nurse can provide:
• Toenail clipping and filing
• Callous and corn evaluation
• A complete foot assessment
• Blood pressure check
• Foot care education
• Coordination with your primary care provider when needed
All feet are welcome, including those requiring special attention because of diabetes, circulatory issues, neuropathy or cardiac conditions.
CVNA also offers At Home Foot Care for Kaiser Permanente Medicare members who are homebound and have a physician referral. A Medical Foot Nurse can provide a comprehensive foot exam, neuropathy screening, circulation checks, blood pressure monitoring, medication review, chronic condition assessment and individualized health coaching and education.
Taking care of your feet is an important part of taking care of your overall health.

Your time can make a meaningful difference
Yesterday, Care Synergy celebrated something truly special: the people who have dedicated years of their lives to caring for others.
At our first annual Service Awards Celebration, we honored 63 employees celebrating 5-, 10-, 15-, 20-, 25- and even 30-year anniversaries. Together, their years of service represent an incredible 555 years of compassion, commitment, care and innovation.
But these milestones are about so much more than numbers. They represent relationships built, families supported, communities strengthened and countless meaningful moments shared along the way.
A highlight of the celebration was the debut of a special video featuring Care Synergy Affiliate employees sharing their stories and reflecting on why their work matters so much to them and to the communities they serve.
One of those employees was Amanda Goodenow, who has worked for Colorado Visiting Nurse Association in multiple roles for 15 years, and currently serves as Director of the National Capable Center.
The words of Amanda and her colleagues are a beautiful reminder that an organization's greatest strength is its people.
To every Care Synergy employee who has made this organization part of your life's work: thank you. We are deeply grateful for your dedication, your passion and the heart you bring to our mission every day.

Today, employees from Care Synergy, The Denver Hospice, Colorado Visiting Nurse Association, and Colorado PACE came together for a shared purpose: giving back to our community through blood donation.
Every blood donation has the potential to save lives, and we're proud of the team members who took time out of their day to help ensure lifesaving blood products are available for those who need them most.
At our organizations, caring for others extends beyond the services we provide. Whether supporting patients, participants, families, or neighbors in our community, we believe in showing up when it matters most.
Thank you to everyone who participated in today's blood drive and helped make a difference.
